During launch, a piece of insulating foam broke off from the external tank and struck the thermal protection system tiles on Columbia's left wing. Similar foam shedding had occurred on previous launches, causing damage ranging from minor to near-catastrophic, but some engineers suspected the damage to Columbia was more serious. When Columbia re-entered the atmosphere, the damage allowed hot gases to penetrate the heat shield and destroy the internal wing structure, causing the orbiter to become unstable and break apart. The bipod ramp foam, which connected the external tank to the orbiter, had been observed falling off on six prior flights and had created some of the largest foam strikes. After the disaster, Space Shuttle flight operations were suspended for more than two years. NASA added on-orbit inspection of the thermal protection system and maintained readiness of rescue missions. Except for one Hubble repair mission, subsequent flights were only to the ISS to allow crew haven if damage prevented safe re-entry.
